your son deserves some privacy, in this case what happenns is they will, stand out beside the door, and wait until your boy does his thing. and yes its called blader shyness. they have no right too stand there and gawek at your kids private parts(exception is strrip searches) he can demand his very privacy.

Faith based or not they need to be sure the urine is coming from your son. So they may have strip to his underwear before going into a stall, or be in the room with you son while urinating. There is no need for them to watch him actually urinate. The minimum would be behind a partition, or have there backs turned to your son if their facilities are limited.
If this not a court ordered stay, then he can talk this through before being admitted.
This sounds stupid, but encourage him to drink lots of water. It will help clean out his system, and make it easier to go.
Also, when he has to go let them know. “I’ve got to take a leak, are you going to want a sample from me today?”. He’ll come across as cooperative, and the staff probably has a certain amount of tests per day/week/month and they’ll appreciate his help.
